build: add -Wimplicit-fallthrough
Also:
- replace all /* fallthrough */ comments with portable fallthrough;
pseudo keyword to accomodate both gcc and clang
- add missing break; statements as required by older versions of gcc
- cleanup some code to remove unnecessary fallthrough
