doc: begin developer's manual

* Move doc/code/ -> doc/developer/
* Move disparate code documentation to doc/developer/
* Convert Markdown docs to reStructuredText
* Organize docs into Sphinx tree
* Move build docs -> doc/developer/
* Change build doc titles to fit under Building subsection of manual
